Direct debit authorisation

I hereby authorise the Shift Up For Rwanda e.V. support association to collect the annual membership fee of €15.00 from my account.

I authorise the Shift Up For Rwanda e.V. support association to collect the payments I am required to make from my account by direct debit when they fall due. Before the first SEPA direct debit is collected, the Shift Up For Rwanda e.V. support association will inform me in writing of the date and the mandate reference number. At the same time, I instruct my bank to honour the direct debits collected from my account by the Shift Up For Rwanda e.V. support association.

Note: I can request a refund of the debited amount within 8 weeks of the debit date. The terms and conditions agreed with my bank apply.
